Attached is the 2010 CCC Awards consideration form. Please please please fill it out and send it in, even if you've only done one or two events - unrecognized or recognized. Click Here to Download the Form (MS Word Doc)
It's important to let us know your results. Even if you think you haven't qualified for a year end recognition, your points go towards your trainers Yearend High Point Trainer award. Help to make your trainer shine, just as he/she helps you!
At the end of the season, print out the form, fill it out and mail it to Wendy (address is on the form).
